Documents required for enrollment of students in grades K-12

• Birth certificate

  • Original for Kindergarten students

  • Copy for students in grades 1-12

• Immunization records (with official stamp from doctor's office) - print out a Physical examination form*

• Copy of student's previous or most recent report card/transcripts. School address and school phone number are also required.

• Proof of residence (three required for incoming kindergarten students)

  • Telephone or electric bill is preferred. Other proofs of residence are bank statements, insurance bill or credit card bill. Must have current address on these documents.

  • If you are building a house, the contract signed by the buyer and seller is required until you receive your first utility bill, which must be submitted.

  • If you are living in an apartment complex and have none of the above, the lease is acceptable until one of the above proofs of residency is provided, which should be within 30 days. 

  • If you are living in an apartment with no lease, a signed affidavit from your landlord is required. Print out a Landlord Affidavit form*

  • If you are living with others, fill out the Living With Others Affidavit form*
     

• Parent's driver's license

• Custody papers (if applicable)

• Individual Education Papers (IEP) for students with special needs (if applicable)
